Certificate of Insurance Request Form

Some venues may require you to submit a Certificate of Insurance (COI), which shows proof of our company's liability insurance. If you need a COI for your event, you will need to fill out this form at least 5-7 days before your event along with a $100 payment. Once we receive the COI, it will be emailed to you promptly.
